IMPORTANT: Completion of these forms in advance and arriving 15 minutes prior to your scheduled appointment time will speed your check-in process and will ensure that your visit runs smoothly. Please have your forms completed before you arrive. If you wait until the time of your visit to complete your forms we will not be able to see you on time.
New patients We ask you to arrive 15 minutesbefore your scheduled appointment time so paperwork can be completed.
If you arrive 20 minutes after your scheduled time, we will almost certainly need to reschedule your appointment .
All patients If your insurance requires a written referral, the referral must be here in the office by the times indicated above. If not, your appointment will need to be rescheduled.
Please be sure to bring the following with you to every appointment: Referral if required Co-pay as dictated by your insurance plan Current Insurance Card Photo ID
Is the Patient a Minor?
Patients under the age of 18 MUST be accompanied by a parent or legal guardianON EVERY VISIT. Minor patients that arrive unaccompanied will not be seen and will need to reschedule when a parent is available.
Dr. Lowitt does not participate in the United HealthCare network. Should you elect to receive your care with us, as a courtesy to you our billing department will submit your claim to United. United would consider the service to be and "Out of Network Visit." We therefore advise that you contact your insurance provider 1) to make sure that your policy includes Out of Network coverage and 2) to see exactly what that coverage entails. Many policies will assign a greater financial responsibility to the patient should they choose and Out of Network provider. If you do elect to be seen here, we will require a signed waiver from you, indicating that you understand that you may be responsible for the portion of the bill that United does not cover.
Referrals If your insurance requires a referral from your primary care physician, please be sure that you bring the referral form with you. If your referral is not in our office at the time of your visit, you will not be seen that day and you will have to reschedule your appointment.
Please be aware that referrals are TIME LIMITED and it is YOUR RESPONSIBILITY to be certain that the dates of your visits fall within the start and end dates on your referral. This applies to new as well as all follow-up appointments. The office staff cannot be responsible for keeping track of the active referral dates. If your appointment is rescheduled for any reason, you must check to be sure that the new date is still within your referral window. The staff will NOT be able to call your primary's office for an updated referral if your referral is not active on the day that you come to be seen. If the referral is not active, you will need to reschedule.
Our office is on the GBMC Campus. The Main Entrance to the campus is at a traffic light on N. Charles Street. Look for the brick sign for GBMC.
NOTE: Patients using a GPS often get lost on their way here.You must enter the campus at the GBMC main entrance.
Once on the GBMC Campus, pass the guardbooth and bear right.
Follow signs for Physicians Pavilion East. Note: You will drive past Physicians Pavilion West and the West Garage - Daffodil Park, which you should NOT use. Continue past this garage and park in the Physicians Pavilion EAST Garage - Iris Park.
This is an enclosed/covered garage.
Cross the street, enter the building and take the first set of elevators to the 3rd floor.
